Patients with obsessive-compulsive disorder (OCD) often receive psychopharmacologic treatments because of an intractable distressing, persistent or pervasive set of obsessions and compulsions. Animal models of OCD have identified behavioral effects following thalamic and striatal lesions and in working memory experiments implicate the prefrontal cortex. This chapter presents a plan for this approach to management of OCD in a resource-limited environment. Conventional OCD medications and behavior therapy are often ineffective. Brain stimulation, including cortical electrodes, deep brain stimulation and the more recently developed optogenetics, are powerful in their capabilities of localizing brain tissue, mapping networks and determining if disruption of neural activity produces therapeutic benefits. The chapter summarizes emerging findings in neurocircuitry in which thalamic and striatal lesions, and functional neuroimaging using positron emission tomography and single photon emission tomography reveal changes in limbic motor cortex coupling, and the ability to document changes in OCD behavior as a consequence of interruption of mesocorticolimbic circuits.
